Carol Cole, M.S.W. , L.C.S.W., MSWAC, SAP (Substance Abuse Professional, Dept. of Transportation), and Board Certified Diplomate, Certified Sports Counselor, Master Social Work Addictions Counselor, Continuing Education Provider and Certified Pranic Healer, offers over 20 years of clinical experience and training with diverse populations.

Ms. Cole is an established national lecturer, and was NASW’s 1997 “Social Worker of the Year” for reg. F. She received The Michael Lambert, Ph.D. 2003 and 2007 Award for Clinical Excellence from PacifiCare Behavioral Health.

She began practice in community mental health in Orange County, CA, where she has been involved in program development and implementation with chronic mentally ill and dual diagnosis populations; Clinical Director of the Orange County Health Care Agency Psychiatric Emergency Team, Adolescent inpatient, Foster Care, Child Protection and Adoptions, as well as Geriatric populations and Trauma Specialist, and is a Certified Master Social Work Addictions Counselor and is a charter member of National Association of Sports Counselors.

Ms. Cole is an Adjunct Professor at USC and has held part-time positions at California State University, Fullerton specializing in Cross Cultural Counseling, as well as serving as a Field Instructor at California State University, Long Beach Graduate School of Social Work; former Clinical Supervisor for Orange County Inpatient Managed Care Hospital Diversion Services; Co-author of Everything You Need to Know To Pass The L.C.S.W. Orals; and author of No Longer A Victim, Practical Guide to DSM-IV Diagnosis & Treatment 1st & 2nd Editions, California and National Mock Written Social Work Exam Study Material, Audio Training CD’s and her latest book, Everything You Need to Know to Become a Competent Clinician.
Ms. Cole is a dedicated exam licensing trainer and CEU provider for Board of Behavioral Sciences and Board of Registered Nursing and National Board of Forensic Counselors, former faculty for NASW, and was nominated for the “Koshland Award” for California’s best practitioner of 1998.

Ms. Cole is in private practice in Temecula, California and is the co-founder of Addictions & Pain Healing Center in Temecula. She and her staff have a Holistic approach to assisting the healing process from Alcohol and Drug addictions, as well as eating disorders, pain, panic and anxiety disorders.

Fees are $125 for cash/check or $130 for credit cards. We also offer TeleHealth (computer-to-computer). In network with Anthem, Blue Shield, CIGNA, IEHP, Magellan, Medicare, Managed Health Networks (MHN), Tricare, Optum – United Health Care, UnitedHealthCare-Military & Veterans.

Appointments

Schedule your appointments online at a time convenient to you. Not only can you make your own appointments instantly, you can also check the date and time of your next appointment, how many times and dates you have seen Carol Cole, and how many appointments you have remaining. You can also go online and change or even cancel your upcoming appointment (at least 24 hours notice, not including weekends and holidays to avoid a missed appointment session full charge. This can be performed on your computer, laptop, notebook or smartphone with an internet connection.

However, if you would prefer to phone and leave a voicemail, that is absolutely fine, as well. Whatever works best for you.

Payment

Carol Cole, LCSW Fee for Service rate is $125 for 45 minute sessions. We accept cash, check, Visa, Mastercard, American Express. There is an additional $5 for credit card payments. Terms are payment or copay at the time of service.

Cancellation Policy: Please arrive at your appointment on time. Sessions will not run beyond your allotted time. If you cannot be present for your scheduled appointment, at least 24 hour advance notice (not including weekends and/or holidays) by email/voice mail to 951-506-0560, or on Schedulicity, is required. Appointments not cancelled within these terms will be billed their regular session rate. Thank you for your consideration.

Carol Cole, LCSW is a contracted provider for the following insurances:

Aetna Healthcare, Anthem Blue Cross, Blue Shield Of California, Cigna Behavioral Health, Cigna Healthcare, IEHP, Magellan, Tricare, Triwest, United Behavioral Health, United Health Care, UnitedHealthCare-Military & Veterans

If you do not subscribe to any of the above mentioned insurance plans, you may have coverage under your plan if it is a PPO and you have met any deductible. Check with your insurance provider for applicability. Should this be the case, and you would wish to see Carol Cole, LCSW, the fee would be $125 – $130 per 45 minute session and Carol Cole’s office will mail you a super bill for you to present to your insurer for possible partial or full reimbursement, depending on the disposition of your insurance carrier.

Should you not recognize any of these insurers, please check with your insurance carrier as one of these companies may be contracting with your insurance carrier for behavioral health coverage. Should you see Carol Cole, LCSW and your insurer does not pay, you will personally be responsible for payment in full to Carol Cole, LCSW at the standard billing rate of $125 per 45 minute session.

Take the 15 Freeway to Rancho California Rd exit and go west (towards the hills/ ocean).
Cross Jefferson/ Old Town Front St, cross Diaz/ Vincent Moraga Ave,
Go up the hill further and turn right at the next light (Business Park Dr).
Continue along Business Park Dr for about 1/8 mile and you will see a open field on the left – then turn right into Glemka Business Parks.
We are the first building on the left as you drive into the office park, closest to the street – Suite 101 (See pictures at bottom of this page).
