| payment Carol Cole, LCSW Fee for Service rate is $125 for 50-60 minute sessions. We accept cash, check, Visa, Mastercard, American Express. Terms are payment or copay at the time of service.
Cancellation Policy: Please arrive at your appointment on time. Sessions will not run beyond your allotted time. If you cannot be present for your scheduled appointment, at least 24 hour advance notice (not including weekends and/or holidays) by voice mail to 951-677-2806 is required. Appointments not cancelled within these terms will be billed their regular session rate. Thank you for your consideration.

Cigna Behavioral Health
Cigna Healthcare
Healthnet
Magellan Health Services
Managed Health Network
Pacificare
Tricare
Triwest
U.S. Behavioral Health Plan, California
United Behavioral Health
United Health Care
If you do not subscribe to any of the above mentioned insurance plans, you may have coverage under your plan if it is a PPO and you have met any deductible. Check with your insurance provider for applicability. Should this be the case, and you would wish to see Carol Cole, LCSW, the fee would be $125 per 50-60 minute session and Carol Cole’s office will mail you a super bill for you to present to your insurer for possible partial or full re-imbursement, depending on the disposition of your insurance carrier.
Should you not recognize any of these insurers, please check with your insurance carrier as one of these companies may be contracting with your insurance carrier for behavioral health coverage. Should you see Carol Cole, LCSW and your insurer does not pay, you will personally be responsible for payment in full to Carol Cole, LCSW at the standard billing rate of $125 per 50-60 minute session. | |
